Paying Dialog Invoices by Credit Card

You may pay your Dialog invoices with your American Express, MasterCard, or VISA credit card. Simply fill out the Invoice Payment Form, sign the form in the space provided, and fax it to 1-734-531-2763.

Instructions:

1. Download the Invoice Payment Form 666 KB
   (Note: The form is a PDF document. To view it, please use the most recent version of the Adobe Acrobat Reader, which is available free from Adobe.)
   
   
2. You can fill out the form online in your browser or print it off and complete it offline. To complete the form online, click on the line following "Name" and type your name. Press the Tab key to go to the next field. Continue to type and tab until you have provided all the required information.
   
   
3. Print the form on your printer.
   
   
4. Sign the form.
   
   
5. Fax the completed form to Dialog Accounts at 919-804-6460.
